
ADMISSION

Exclusionary Criteria:

Significant cognitive impairment (e.g. Alzheimer’s, dementia, intellectual disability, organic disorder)
-
Inability to ambulate independently
-
Inability to complete activities of daily living independently
-
Recent, ongoing serious and violent behavior(s)
-
Sexualized maladaptive behavior(s)
-
Sexual offender (convicted or pending conviction)
Admission Criteria:
-
Age 18 years of age and older
-
AHCCCS or AIHP recipient with an identified health home
-
Willing to take prescribed medication
-
Diagnoses with at least one behavioral health diagnosis; substance use disorder;
-
dual-diagnosis; or be designated as seriously mentally ill (SMI) by the State of Arizona
-
Bed availability

Once a member is approved for admission, the following documents are required before they are physically accepted into one of our facilities:

Completed referral checklist
-
Health Home’s comprehensive psychosocial assessment or annual update
-
Health Home’s current treatment plan with a signature from the behavioral health provider (BHP/licensed clinician) Most recent medication list with an electronic signature from the provider and/or physical scripts

Copy of negative TB/PPD screening or x-ray showing no sign of TB
-
The Chalet House medical clearance/physical form
-
Court-ordered paperwork, if applicable
-
Discharge plan from discharging facility, if applicable

What to bring?
-
7 days worth of comfortable, weather-appropriate clothing to include undergarments, socks, two pairs of shoes and if applicable one pair of house shoes (e.g. slippers)

Female- clothing must be appropriate, not provocative or transparent
-
All- clothing must be appropriate to include not being gang-related, glorifying drug use or politically themed

Specialized personal hygiene and beauty products

General hygiene products are provided

All razor/shavers, hairdryers, curling irons, or electric shavers will be checked for safety

Appropriate reading materials (books and magazines)

Tobacco and/or nicotine products for the duration of
your stay-
We do allow tobacco or use of nicotine products every 2 hours from 8 am-8 pm; however, we will not provide tobacco or nicotine to you.

What not to bring?

It is recommended that you lock up any valuables. The Chalet House staff is not responsible for your valuables.

Television sets, radios, electronic devices such as video game consoles, video games, DVD players, etc.

Drugs, alcohol, non-prescribed medication, aerosols

Guns, knives, other weapons

Pornography
-
Toiletries or beauty products that contain alcohol in the first 5 ingredients

Unapproved, non-prescribed opened over-the-counter medications

Sporting equipment

Revealing clothing including overly tight clothing

Cleaning supplies
-
Candles and incense
-
Clothing with profanity, gang-related, politically themed or glorifying rug use
Outside foods and drinks are also limited because The Chalet House provides three meals a day, snacks, sugar-free and caffeine-free beverages. If you have special dietary needs please notify The Chalet House staff as soon as possible by requesting an order be sent from your primary care physician or allergist.
